HELPFUL HINTS FOR FILLING OUT YOUR "MAIL" ORDER FORM   On the front side of the form, please place the quantity of products you desire in the little white boxes in the "ORDER HERE" area.   Packages     A     B     C     Set of 8 wallets All of our packages are ordered in this area. Packages cannot be split between more than one proof. All 8x10 and 5x7 prints in ANY of the packages come with folders, but you must indicate if they are to be laminated, or non-laminated. Wallets do not come laminated.   Additional sets of wallets can be added to any of the above packages. a la carte Choices     8x10     5x7     wallets Individual prints may be ordered in the a-la-carte section of the form. Indicate how many of each size print you desire. Folders are automatically included with 8x10 prints. You must indicate if you want to order 5x7 prints with or without folders. Be sure to fill out the back side of the form with engraving information.     Class Overview Photograph Class overview. This product is only available if the words "Class Overview" are printed on your order form.   REVIEW THE BACK OF THE FORM BEFORE MAILING Once you have filled out the white boxes and have carried across the total price for each product ordered, you need to total your order. Be sure to add sales tax, shipping and handling, international shipping if applicable, and the $5.00 late fee if you are ordering after the deadline date printed above your name on the order form.
